Registration and Disenrollment Information

2014 - 2015 Registration

Steps to process registration:

  1. Obtain registration packet from school office.
  2. Complete all paperwork.
  3. Submit registration packet along with supporting documents to school office.
  4. Once registration is completed and submitted the student will be able to begin school one full day later.
  5. See specifics below for students who are new to APS and those who will be processing a transfer from another APS school.  

Paperwork Needed:  Birth Certificate, Shot Records, Proof of Residence (PNM bill, Gas bill, Lease or Purchase Agreement), if Native American their CIB, copy of most recent IEP and name and address of school attended last year.  If you are living with family or friends we will need their proof of residence and they will need to accompany you to fill out the proper paperwork. 

Students New to Albuquerque Public Schools

Documents Needed:

  1. Birth Certificate
  2. Current Shot Records
  3. Name and Address of previous school attended
  4. Copy of most recent IEP if student requires special services
  5. Proof of Residence
    (Note:  Proof of Residence can be PNM bill, Gas bill, Lease or Purchase agreement)
  6. If you are living with friends or family who reside in our attendance area:
    1. The owner or renter of the property where you live  MUST accompany you to registration with a 2 forms of their proof of residence (PNM, Gas Company, Lease/Purchase agreement and another piece of mail addressed to them).
    2. The owner/renter will need to be present to complete the proper paperwork needed to confirm the proof of residence.
    3. If they are unable to be present  the required  form will need to be completed, notarized and returned to the office along with their proof of residence in order to complete registration.
    4. The person registering their child will need 2 documents mailed to the address in the parents name.
  7. Emergency contact phone numbers, doctor, dentist, hospital and health insurance information.
  8. If the student takes medication at school, a permission to administer medication form must be completed by Student’s doctor.  Download / view the medication authorization form.

Students Transferring from another Albuquerque Public School

Documents Needed:

  1. Disenrollment paperwork from previous Albuquerque Public School.(You must disenroll from previous school and clear student accounts)
  2. Name of previous Albuquerque Public School.
  3. Copy of current IEP if student requires special services.
  4. Proof of Residence    (Note:  Proof of Residence can be PNM bill, Gas bill, Lease or Purchase agreement)
  5. If you are living with friends or family who reside in our attendance area:

Requirements to attend school once registration paperwork is complete

  1. All documentation must be in order, along with the completed registration packet provided by the school office in order for the student to begin attending school.
  2. Student must have all required immunizations in order to attend school.
  3. Students who will begin Kindergarten must be 5 years old by August 31st.

Disenrollment Information

If you are moving outside of Sombra's attendance area, please be sure you have addressed the following prior to your student's last day at Sombra.

  1. Contact the school office to set up a time to process the disenrollment, when you contact the school you can begin the process by asking if your student has any outstanding items in the library, classroom or an outstanding balance in the cafeteria.
  2. When you come to the office to disenrolled you will be given a disenrollment form which you will take to the cafeteria, library and classroom in order to verify that your student's accounts are cleared before leaving the school.  (Note:  if your student has outstanding items in the cafeteria, library or book room, their negative balances will follow them if you are transferring to another APS School.)
  3. It is helpful to inform the teacher prior to your student leaving so they will be able to collect your student's belongings for their last day.  It will also allow your student to say their good byes to their classmates and friends. (NOTE: This does not replace the disenrollment process, you will still need to process the disenrollment in the school office.)

If you are moving out of town, please have the name of the school the student will attend once your move is complete.  If you do not have the school name the city and state you will be moving to will be helpful.
