Health Services

An APS School Nurse is at Sombra del Monte part of each school week. Sombra’s Health Assistant is on duty in the Health Office each day during school hours.

Immunizations: New Mexico law requires that students be up-to-date on their immunizations BEFORE they can register and attend school. This also applies for students who apply for the Exemption from Immunizations with the State of New Mexico.

Medication Policies: Prescription medications may be administered at school, BUT only with a signed APS Physician's Order form. Over-the-counter medications require a signed APS Parental Authorization form. Parents may obtain these forms in the Health Office. Prescription containers must have a pharmacy label with the name of the drug, strength, dosage, student's name, and time of administration. All medications MUST be kept locked in the Health Office and parents have to pick up remaining medication at the end of the treatment.

Illnesses: Students with a temperature of 101 degrees or greater will be sent home. In other cases the Health Office staff will determine when the parent will be called. If you want to be notified of every health room visit, or if you have other special instructions, you must notify the Health Office in writing for the current school year.

Injuries: In the event of injury on the school premises the child is to report to the duty teacher or Health Office. Parents need to keep emergency numbers updated with the school office and health room for proper notification of illness or serious accident. If parent or emergency contacts cannot be reached and the condition requires immediate medical attention, “911” will be notified. The child will be taken to a medical center designated by the emergency unit. Parents will be responsible for all expenses.

Student Insurance: The Board of Education makes a student insurance policy available to all APS students. All premiums are to be paid by parents/guardians. Forms are available are sent home the 1st day of school and in the school office during the school year. The Health Office staff can assist families in accessing Medicaid information.

Health Screenings: The Health Office will be performing mandatory grade level screenings annually. Parents will be notified of results. Please notify the Health Office if you prefer not to have your child screened.
